Word finding difficulties are unfortunately a common struggle for many brain injury survivors. In a previous post Aphasia from brain injury, I tried to define how language skills can be affected. It was my effort at trying to broadly explain to observers what is happening and how they can help. But now I would like to do something different – explain my experience of it, and the knock on effect it has on my behaviour.

“You know, the thingy…….”

Most people have at some time struggled to put their finger on a name or term. It’s normal. Your brain holds it’s own huge library of words. Think about it, if one book gets put back in the wrong place even the best librarian needs extra time to locate it. Ever wondered how big your library is?

“A reasonably educated person about 75,000……An ordinary person, one who has not been to university say, would know about 35,000 quite easily.” – Professor David Crystal, known chiefly for his research in English language studies and author of around 100 books on the subject.

To read more about estimated vocabulary see the article I took this quote from on BBC News, magazine.

Might as well be a foreign language

But imagine struggling to put a sentence together. It feels more like when your French Teacher is trying to get you to put a word you have just learned into a workable sentence. You sort of know what you mean, but how do you get it out? It all takes so much effort.

Personally I never could cope with French grammar, so I moved over to German instead. Turns out that as long as you avoid using the word AND by starting a new sentence, they build their sentences they same way as we do in English. You don’t have to play hide and seek with the verb, or whatever it is that happens. So when I’m starting to struggle with my words, I start trying to find easier ways to construct sentences, like avoiding And or Und in German. But it doesn’t feel natural. That’s not the way people talk. So even as I’m trying to do this jiggery pokery in my head, I come to realise it doesn’t sound right.

Is it important enough for this amount of effort?

Then I start to lose the will to continue trying. Perhaps I’m lazy, but when I’m tired anyway I quite often give up playing the find the word game. Instead I might just say “I don’t know”, which isn’t entirely true. But because I don’t want to continue, it’s my get out of jail card.

I still remember being the teenage know it all who would have done anything to avoid ever having to say “I don’t know”. She would be ashamed of me. She does scold me a little bit every time, but who ever listened to a spotty brat with holes in her tights anyway. But she does have a point. As the other person is then put in the position of trying to educate me, I get even more frustrated. Yes I know all this, I just wish you could plug me into something and just let it all download.

All of this equals stress, which only exacerbates my tiredness. It feels like a lose lose situation. It is not anyone else’s fault, but my own. I need to be more patient with myself, or just be honest and say I need to reboot before we can continue.

But after my head injury it had got even more ridiculous. My partner James felt the need to walk behind me when going up stairs, and in front of me when going down so if I fell he could cushion me. But it wasn’t just the stairs we had to worry about.

Lots of people have accidentally hit their head on a open cupboard door but usually they learn from the experience. I didn’t.

Left unsupervised I could get myself into all kinds of trouble. Stupid things like I might take the lightening cable out of my phone that had been charging over night, but drop the phone through the open iron head board. Reaching down to retrieve it, I would forget a little thing like there being a wall, and smack my head.

Once you have a head injury, any time you hit your head it feels like you’ve gone back to where you started.

Every time a had a silly accident like this it hurt like hell. I’m not talking about the bump or bruise, I’m talking about feeling like your head is going to explode. If you have ever seen one of those films where someone gets trapped in a decompression chamber and the other guy throws the switch so the person inside explodes, the moments before they die their face explains it all.

The worst one was the accident I had with a revolving door.

I’d gone to see some friends who’s office had moved into a posh service centre. I’d been there before so I knew what to expect. Perhaps if it was because I was thinking about where I needed to be, but this time as I left I got it all wrong.

<img loading="lazy" decoding="async" data-attachment-id="2783" data-permalink="https://www.jumbledbrain.com/mind-your-head/why-you-must-mind-your-head-after-brain-injury/" data-orig-file="https://i0.wp.com/www.jumbledbrain.com/wp-content/uploads/2016/09/Why-you-must-Mind-Your-Head-after-brain-injury.png?fit=800%2C1200&ssl=1" data-orig-size="800,1200" data-comments-opened="1" data-image-title="Why you must Mind Your Head after brain injury" data-image-description="" data-image-caption="" data-large-file="https://i0.wp.com/www.jumbledbrain.com/wp-content/uploads/2016/09/Why-you-must-Mind-Your-Head-after-brain-injury.png?fit=580%2C870&ssl=1" data-recalc-dims="1" class="aligncenter size-full wp-image-2783" src="https://i0.wp.com/www.jumbledbrain.com/wp-content/uploads/2016/09/Why-you-must-Mind-Your-Head-after-brain-injury.png?resize=580%2C870&ssl=1" alt="After a brain injury the last thing you need to do is have a bang to the head, so you must take care.
I didn’t notice that there wasn’t a open space and that it needed to spin a little more. I just walked straight into the glass. Wham. But I was so busy trying to regain my composure and dignity that I just carried on. The pain was unbearable, but I got into my car ready to leave. I reversed out, and started driving past the other parked cars. Then someone else started to back out, and it seemed they hadn’t spotted me yet. I knew I had to respond, but as I’d just bashed my head so hard I just couldn’t think fast enough how to stop the car! So I stepped harder on the accelerator hoping I could get out of the way fast enough.

I was lucky this time to get away with it. But it did scare me how I couldn’t remember how to move my leg to hit the break.

Multiple concussions are very dangerous. The brain finds it harder to recover from multiple injuries particularly when it is still working on one. So if you have or know someone who has had a head injury, please look out for them. The brain is as precious as it gets, and so far a transplant isn’t possible.

After a head injury you need to make sure you are fit to drive again.
